Your samples must be labeled, packaged and mailed to our
lab. A submission form must be mailed or emailed before analysis.
Print a Form
Download and print the sample submission form.
Complete all personal, animal, and testing information.
Clearly label every blood vial and label it to match submission form.
- Tube # (1, 2, 3, 4, 5, etc)
-Animal ID (Tag #, or name)
Include the completed form with payment information inside your package.
Digital Form
Download the sample submission form.
Type details into the form fields.

Be sure to include your name and phone number inside your sample shipment so we can match your submission to the digital form.
Before Mailing Your Samples
